Health Education Seminars

These seminars provide the community with valuable knowledge on a range of health topics, such as chronic disease prevention, mental health awareness, and preventive care practices. Developed by a primary care physician, these educational sessions aim to empower individuals with the tools they need to make informed decisions about their health, improve wellness, and reduce risk factors for common diseases.

Health Screenings

In an effort to identify undiagnosed illness and identify those at increased risk for developing illness, health screenings including blood pressure checks, cholesterol and glucose testing, and body mass index (BMI) measurements among others allow individuals to take proactive steps in managing their health and prevent the progression of conditions, such as hypertension or diabetes.

Healthy Eating Program

As primary care physician developed, community-based program focused on promoting healthy eating habits, this program offers education on balanced diets, nutrition labels, portion control, and meal planning strategies. Participants also receive practical tips for managing chronic conditions (e.g., diabetes, hypertension) through diet, and may receive cooking demonstrations or grocery shopping guidance to help them make healthier food choices in everyday life.

Health Advocacy

Physicians act as health advocates for community members, helping them navigate the healthcare system, access necessary services, and ensure they receive timely, appropriate care. Services such as assisting with insurance questions, educating patients and care givers about specialty care, and providing guidance on how to access health resources or special programs support the well-being of the individual and the community.

Health Coaching for Special Populations

This service provides individualized health coaching for groups with specific needs, such as individuals with chronic illnesses (e.g., diabetes, heart disease), seniors, pregnant women, or those seeking weight management. Through one-on-one sessions, the primary care physician offers tailored advice, goal setting, and continuous support to help clients better manage their health, improve lifestyle choices, and prevent complications related to their specific conditions.

Health Asset Access

This service connects community members to available health resources, services, support systems, health devices and medications to improve the overall health of the community. Access to these “health assets” ensures the community at large can benefit from having the care and resources they need to manage their health and improve their overall quality of life.
