Blood Pressure

After clinicians review relevant lab results (e.g., blood pressure readings) and patients enter their home monitoring data, a virtual group session is used to deliver physician-developed education on the general causes, risks, and effects of high blood pressure, as well as lifestyle modifications and medications used to help manage the condition as well as monitoring benchmarks. If necessary, additional individual sessions are utilized to focus on adjustments to the personalized management plans and monitoring protocols.

Blood Sugar

Patients learn about the factors influencing blood sugar levels, how to interpret lab results (e.g., HbA1c), and the importance of diet, exercise, and medication in managing blood glucose. The group session covers general guidelines and standard management, while individual virtual sessions offer specific advice for patients with unique situations or unexpected responses to the prescribed regimen.

Cholesterol

After reviewing patient-entered data and lab results (e.g., lipid profile), patients are educated in a group setting on cholesterol’s role in cardiovascular health, the different types of lipids (e.g., LDL, HDL), and how diet, exercise, and medications (statins, etc.) can help manage lipid levels. If necessary, individual sessions can be added to provide tailored recommendations on medication, dietary adjustments, and long-term strategies to optimize cholesterol management.

Weight Management

After entering data such as current weight, current and previous eating plans, and exercise habits, as well as health goals, patients participate in group sessions that educate patients on the essential elements of weight management, including the role of nutrition, exercise, and behavioral modifications. Using the patient data and relevant lab results a customized weight-loss plan is formulated with recommendations for medications, lifestyle adjustments, and ongoing monitoring of progress. Regular monitor, visits and adherence to prescribed treatment plan are required to maintain good standing in this care group.

Anemia

In the virtual group session, patients are educated on the types of anemia (iron-deficiency, B12, chronic disease, blood loss, etc.), symptoms, and potential causes. After reviewing patient-entered data and lab results (e.g., CBC, iron studies), personal care plans are formulated that address the specific cause. If necessary, individualized sessions dive deeper into specifics unique to the individual and special concessions that may be needed to best address and correct deficiencies or manage underlying conditions.

Fatigue

A group session addresses common causes of fatigue, including lifestyle factors, stress, sleep issues, and underlying medical conditions. Patient-entered information such as sleep patterns, activity levels, and biometrics are considered with the results of an extensive lab panel and other studies to determine the cause of the fatigue. Depending on the cause, the patient will receive a care plan tailored specifically to address the cause of the fatigue. Depending on the cause, the patient may also be referred to another care group.

Testosterone

The group session covers the basics of testosterone and its role in men’s health, including signs of low testosterone, normal levels, and treatment options (e.g., injections, creams, pellets). After lab work (e.g., testosterone levels, estradiol), the clinician reviews individual data to personalize a treatment plan, assign appropriate monitoring protocols, and address potential side effects or health concerns. Regular monitoring and visits are required to maintain good standing in this care group.

Compassionate Use

Patients in need of alternative treatment options for serious conditions may benefit from compassionate use protocols. In a group setting, patients are informed about compassionate use programs and eligibility criteria for accessing medications outside of traditional clinical trials. Individual virtual sessions then address specific needs, ensuring proper documentation, eligibility assessment, and the safety and monitoring of treatment under compassionate use provisions.
